廖洪躍副主任醫(yī)師 南昌大學(xué)二附院 醫(yī)療美容科 一、適應(yīng)范圍厚唇變薄、薄唇變厚、口裂縮小、口裂增大、M型微笑唇、唇紅邊緣不齊、唇弓不鮮明、唇紅局部缺口、唇外翻、唇系帶過短、唇形矯正二、術(shù)后注意事項(xiàng)1、護(hù)理1-2天內(nèi)唇部避免過分夸張表情,盡量少說話。注意保持口腔的清潔，每天要用漱口液，如硼砂、多貝爾氏液，含漱四到六次;手術(shù)以后要嚴(yán)格地遵守醫(yī)生囑咐服藥，并且及時(shí)就診。,2、飲食手術(shù)后吃流質(zhì)食物三天，不吃太燙的食物，常溫即可。術(shù)后一周內(nèi)忌煙酒、魚蝦、辛辣刺激食物。吃完東西及時(shí)用生理鹽水清潔唇面，保持囗腔與唇部清潔，以免感染發(fā)炎。3、預(yù)防感染術(shù)后一般不需要包扎敷料，手術(shù)切口縫合后按醫(yī)生叮囑用抗生素軟膏涂抹，口服抗生素。4、換藥、拆線術(shù)后5-7天拆線，拆線后24小時(shí)內(nèi)傷口不能沾水；拆線后避免過多的唇部活動(dòng)，太多的唇部活動(dòng)可引起瘢痕增生，甚至出血腫脹。5、出血術(shù)后當(dāng)天傷口可能出現(xiàn)少量滲血，屬正?，F(xiàn)象，但如出現(xiàn)出血不止、術(shù)區(qū)劇烈腫脹疼痛應(yīng)立即到醫(yī)院進(jìn)行急診處理。6、止痛一般無需服用止痛藥，個(gè)別體質(zhì)者可在當(dāng)天夜間給予口服2-4片洛芬待因緩釋片。一般第2天即無明顯疼痛感。7、消腫一般術(shù)后2-3天時(shí)水腫厲害，大部分在1周后逐漸消退。8、預(yù)防瘢痕術(shù)后不可用力揉搓傷口，可涂抹祛疤軟膏淡化疤痕。9、其他術(shù)后2-3月之內(nèi)切口可能略紅，觸之稍硬，偶有瘙癢，之后逐漸恢復(fù)，穩(wěn)定后一般社交距離不易看出。術(shù)后可能會(huì)有上唇白唇麻木、感覺遲鈍等現(xiàn)象，一般半年左右逐漸恢復(fù)。建議術(shù)后正常活動(dòng)，但不要進(jìn)行強(qiáng)度較大的體育鍛煉。健身、游泳等活動(dòng)建議等到術(shù)后1個(gè)月之后。10、復(fù)診術(shù)后1周、1月、6月需返院復(fù)診，醫(yī)生將據(jù)此判斷手術(shù)效果并將給出后續(xù)處理建議，請事前與醫(yī)生預(yù)約并按時(shí)復(fù)診。2020年06月16日3224

Do not make up.4、準(zhǔn)備術(shù)后用藥：冰袋2-3個(gè)、消腫藥（1周量）、止疼藥（1-2次）、外用祛疤膏（3月量）、漱口水（1周量） Medication preparation after surgery: ice pack, painkillers (on the night after surgery), detumescence medicine (within 1 week after surgery), mouthwash (within 1 week after surgery)術(shù)后注意事項(xiàng)Post-operative Care1、術(shù)后立刻唇部冰敷30-60分鐘，之后3天內(nèi)每天冰敷3-4次，每次30-60分鐘 Ice compress on lips between 30 and 60 minutes should be done immediately after surgery, and then ice compress 3 or 4 times a day between 30 and 60 minutes each time should be conducted within 3 days thereafter.2、術(shù)后當(dāng)晚可能術(shù)區(qū)會(huì)比較疼痛，必要時(shí)可以服用止疼藥或安眠藥幫助睡眠。 Antibiotics are not required unless otherwise requested. Pain may be severe on the surgery day or the first day after surgery. You may take painkillers before sleep if necessary.3、術(shù)后前3天進(jìn)軟流食，避免過熱食物，進(jìn)食后注意漱口。1周內(nèi)唇部避免夸張表情，盡量少說話，避免大笑以及用力哈欠。Regarding post-surgical diet: the cold liquid diets should be taken on the surgery day. The soft food is preferred within 1 week, and tobacco, wine, fish and shrimp, spicy and irritating food should be avoided. Mouthwash should be used after meals, and local movement should be avoided to the maximum extent.4、術(shù)后7天拆除上唇邊緣縫線。Suture will be removed 7 days after surgery. Keep the incisioin clean before stiches removal.5、術(shù)后術(shù)區(qū)出現(xiàn)水腫、青紫、淤血等，屬于正?，F(xiàn)象，一般在術(shù)后2-3天時(shí)水腫最厲害，可服用消腫藥。大部分在1-2周后逐漸消退，如發(fā)現(xiàn)腫脹淤血或疼痛加劇、出血等等，請即聯(lián)系醫(yī)生。Edema, bruising or congestion in the post-surgical area is normal phenomenon. Generally, the edema is the most severe between 2 and 3 days after surgery. If you want the edema to be subsided quickly, you may take the detumescence medicine for about 1 week from the surgery day. Most of them will gradually subside after 1 or 2 weeks, but if swelling, congestion, increased pain or bleeding is found, please contact the doctor immediately.6、水腫時(shí)可能會(huì)發(fā)生個(gè)別縫線脫落可能，一般不致傷口裂開，所以不必驚慌，將縫線吐出即可。個(gè)別萬一發(fā)生傷口裂開者請立即聯(lián)系醫(yī)生。 Some sutures may fall off when edema occurs. Generally, it will not cause an open wound, so please do not panic, and it will only need to take out the suture. Contact a doctor immediately in case of open wound.7、術(shù)后瘢痕組織軟化和淡化需要大約3-6個(gè)月時(shí)間，這段時(shí)間內(nèi)有可能會(huì)出現(xiàn)切口疤痕硬、鼓起，笑不開，表情不自然，僵緊感，左右不對(duì)稱，黏膜脫屑等，請耐心等待，一般隨瘢痕組織逐漸軟化而緩解。 It need 3-6 months for the incision scar to recover. During this period, stiff sense and unnatural expression may occur.8、由于術(shù)后兩側(cè)恢復(fù)速率可能存在差異，短期內(nèi)輕微不對(duì)稱可能比較常見，一般徹底恢復(fù)后能逐漸不明顯；另外個(gè)別案例術(shù)后可能存在局部欠平整，一般隨疤痕軟化后會(huì)緩解流暢，最終效果可在術(shù)后至少3月以后再評(píng)價(jià)，如果存在矯正不足或過度、不對(duì)稱或者局部不滿意，如徹底恢復(fù)后仍存在可在半年后再次修復(fù)。 Asymmetry may occur in a short period after surgery due to the inbalance recovery. Intravenous medication is applied without endotracheal intubation, and there will an additional charge (between 1,500 and 3,000 RMB subject to surgical time).術(shù)前準(zhǔn)備 Preparation before lip surgery1、常規(guī)檢查：(可手術(shù)當(dāng)天做，局麻者無需空腹，靜脈全麻者空腹化驗(yàn)；外地患者為節(jié)省時(shí)間也可以先在當(dāng)?shù)厝?jí)醫(yī)院提前完成，3個(gè)月內(nèi)有效) Regular test:局麻手術(shù)者：血常規(guī)、尿常規(guī)、凝血四項(xiàng)、美容免疫四項(xiàng)（乙肝、丙肝、梅毒、艾滋）；For localanesthesia:routine blood test，routine urine test，coagulation function，screening of infectious diseases靜脈全麻手術(shù)者：上述6項(xiàng)、肝腎功能、空腹血糖、心電圖、胸片F(xiàn)orgeneral anesthesia: The above andblood biochemistry，ECG，chest X-ray2、選擇局麻者正常飲食，選擇靜脈全麻輔助者術(shù)前6小時(shí)禁食、4小時(shí)禁水 A 6-hour ambrosia and a 4-hour water deprivation before surgery are required for patients under general intravenous anesthesia.3、術(shù)前注意術(shù)區(qū)清潔，剪短鼻毛與胡須，勿化妝 Keep the face clean, cut thenasal hair and beard. Do not make up.4、準(zhǔn)備術(shù)后用藥：冰袋2-3個(gè)、消腫藥（1周量）、止疼藥（1-2次）、外用祛疤膏（3月量） Medication preparation after surgery: ice pack, painkillers (on the night after surgery), detumescence medicine (within 1 week after surgery), mouthwash (within 1 week after surgery), anti-scar ointment (from the 3rd day after suture removal), anti-inflammatory medicine (if necessary).術(shù)后注意事項(xiàng) Post-operative Care1、術(shù)后立刻唇部冰敷30-60分鐘，之后3天內(nèi)每天冰敷3-4次，每次30-60分鐘 Ice compress on lips between 30 and 60 minutes should be done immediately after surgery, and then ice compress 3 or 4 times a day between 30 and 60 minutes each time should be conducted within 3 days thereafter.2、術(shù)后當(dāng)晚可能術(shù)區(qū)會(huì)比較疼痛，必要時(shí)可以服用止疼藥或安眠藥幫助睡眠。 Antibiotics are not required unless otherwise requested. Pain may be severe on the surgery day or the first day after surgery. You may take painkillers before sleep if necessary.3、 當(dāng)天進(jìn)冷流食。 2天后正常飲食，但術(shù)后1周內(nèi)忌煙酒、魚蝦、辛辣刺激食物。Regarding post-surgical diet: the cold liquid diets should be taken on the surgery day. The soft food is preferred within 1 week, and tobacco, wine, fish and shrimp, spicy and irritating food should be avoided. Mouthwash should be used after meals, and tooth-brushing should be avoided to the maximum extent.4、術(shù)后5天拆除全部縫線。切口在拆線前保持清潔，可以涂抹紅霉素軟膏保持濕潤。Suture will be removed 5 days after surgery. Keep the incisioin clean before stiches removal.5、1周內(nèi)唇部避免夸張表情，盡量少說話，避免大笑以及用力哈欠?？梢运⒀?，但注意輕柔，勿觸碰傷口。 Within 1 week after surgery, the exaggerated expressions on lips should be avoided, please speak as little as possible, and laugh and yawn should also be avoided.6、 術(shù)后術(shù)區(qū)出現(xiàn)水腫、青紫、淤血等，屬于正常現(xiàn)象，一般1-2周逐漸消退，如希望腫脹盡快消退可在術(shù)前1天開始服用消腫藥。 Edema, bruising or congestion in the post-surgical area is normal phenomenon. Generally, the edema is the most severe between 2 and 3 days after surgery. If you want the edema to be subsided quickly, you may take the detumescence medicine for about 1 week from the surgery day. Most of them will gradually subside after 1 or 2 weeks, but if swelling, congestion, increased pain or bleeding is found, please contact the doctor immediately.7、個(gè)別案例拆線后可能出現(xiàn)切口表層裂隙、局部小缺損或者結(jié)痂等，一般3-5天自行恢復(fù)，可局部涂抹紅霉素軟膏；個(gè)別案例可能出現(xiàn)皮下可吸收線（白色線結(jié)）外露，可聯(lián)系醫(yī)生拆除。 Some sutures may fall off when edema occurs. Generally, it will not cause an open wound, so please do not panic, and it will only need to take out the suture. Contact a doctor immediately in case of open wound.8、外用祛疤膏可在拆線愈合后3天開始用，待祛疤膏干結(jié)形成保護(hù)膜以后，表面可以化妝。瘢痕一般在術(shù)后3-6個(gè)月左右穩(wěn)定，盡量減少夸張表情活動(dòng)，以及外用祛疤膏預(yù)防。如仍較明顯可能需要進(jìn)一步治療：鼓起增生者可以疤痕內(nèi)注射含激素的祛疤針，平坦或者凹陷的疤痕可以后期激光減淡。Anti-scar ointment is recommended externally 3 days after suture removal, and continuously applied for about 3 months. If there is dense hyperplasia scar above the lip surface, the steroid hormone can be injected locally (once a month) for softening.9、由于術(shù)后兩側(cè)恢復(fù)速率可能存在差異，短期內(nèi)輕微不對(duì)稱可能比較常見，一般徹底恢復(fù)后能逐漸不明顯；另外個(gè)別案例術(shù)后可能存在局部欠平整，一般隨疤痕軟化后會(huì)緩解流暢，最終效果可在術(shù)后至少3月以后再評(píng)價(jià)，如果存在矯正不足或過度、不對(duì)稱或者局部不滿意，如徹底恢復(fù)后仍存在可在半年后再次修復(fù)。 Asymmetry may occur in a short period after surgery due to the inbalance recovery.
